I hereby expressively and affirmatively state that I am choosing to participate in playing in the kids zone jump and play area at Southern Hill Farms Market. All activities in the area are covered by signing this consent form, including but not limited to: Bounce Houses, Obstacle Course, Jump Pillows, Mechanical Swings, Giant Fun Slide, Carousel, Pirate Ship Swing, Berry Trooper, Papa's Roadsters, Berry Go Round, Rock Climbing Wall, Dirt Mound Climb and all water activities. I realize that participation in these activities is at my own risk. I agree that Southern Hill Farms Market will not be held responsible or liable for any property damage, loss, injury or death of any nature while participating in these activities. If a child/children under the age of 18 are in attendance, I am signing on their behalf and will assume all responsibility, liability or risk for them as the legal adult. All children under the age of 12 will be supervised by a guardian or parent at all times. I am voluntarily choosing to participate and assume all responsibility, liability and risk associated with this activity.
